Have you tried this....
Unroot first via Super SU
Odin OC5 Tar
Let phone boot up
Power phone down via Press and Hold of Power Button
Boot into factory recovery( Press and hold Power button+ Vol Up + Home Button. When phone reboots you should see Recovery is Booting. Release power and home button while still holding Vol Up)
Wipe Factory Data and Cache as well
This will take phone to stock out the box with everything reading Official
Reroot phone and then flash this rom
You should be good to go after these steps.

I had to flash the NK4 baseband. So I am on stock NK4 with all of my old set up. So I should be able to flash the OC5? Flas the OC5, then flash TWRP and this will give me root. Then flash this ROM, Correct?
I do not have ROOT on the NK4 at this time

Once on nk4 you should be able to take the Ota update to oc5 then proceed with the rooting and recovery process
WooHoo finally got OC5 to work. Had to do a factory reset once I installed it. Now I have this running. Thanks for all the help.
Hi I'm loving the rom! I've been using titanium backup pro to freeze apps and processes. BUT, I went to far and got while trying to access the Settings app: 'Unfortunately, Settings has stopped.'
I went through and defrosted most of the items and got it back.
I still don't know which one caused it.
Anyone got an idea of which one it was?
Or is there a sprint note 3 lollipop bloatware that actually is up to date?
Most of what I've seen is for KitKat or for the s5.
Is there a way to export the list of what I've frozen from titanium?
I'll be over the next few days freezing applications/processes and trying out which one causes setting to go crazy.
Any speculations?

Anyone have a link to some flashable themes that work for this ROM? Searched this thread and I am having trouble finding any.
To the best of my knowledge there are no themes available for this rom. I am in the process of making a custom rom based off this rom.

BTW: Tried Sacs Dark Lord V3 port for a couple days and came back to stock rooted. Loved the s6 theming features but had a few minor glitches that forced me to come back.

To find frozen apps manually.
Look for certain files... packages.xml and package-restrictions.xml
Inside those files look for the part of a string "enabled=2" whatever app has 2 is frozen. Just change 2 to 1. That is one way that I can think of.

Johnny why not make a thread for your rom? Wouldn't that be better then promoting your rom in this thread? It might be confusing to some who come in here and expect that this theme is part of this rom and its not.
Wouldn't it also be easier for you as well?
