I've never seen such an issue, as I told earlier.
I think you'd try a "clear dalvik cache/cache" option from recovery and see.
If the issue remains, I'd suggest you to wipe everything and reinstall.

I've applied the patch you provided and straight away it's better on earpiece volume. The issue I have now is receivers of a phone call are complaining that there is an echo when I'm talking.
I don't mind that much as I tend to use wired earphones with a built in microphone but it would be nice to not have echo's when I'm using the phone.
This fix is now included by default in 20161228
From 20161228 changelogs are automatically generated.
yes, since this is just the same crappy CM 14.1 as we all used before with echo, fingerprint issues, lag, sRGB mode not working, battery drain, no dash charge indicator, no forceencrypt, camera hal issues, hotspot issues...

@koenkk boss, may I point you to oneplus 3 thread below where it had build in echo fix + gps from the experimental commits.
I Will be upgrading to a 3t soon and I can confirm that with the commits, there is no echo and GPS fixes like stock oos.

In my opinion, the problems with the fingerprint sensor are very small, in most of the times it works without problems.
I wouldn't call not working at all unless I unlock my device every couple of times , small.
Tethering workaround
In the lineage-14.1-20161230-UNOFFICIAL-oneplus3 ROM, tethering was successfully enabled using this procedure from the Nexus 6 forum
I ignored the unroot parts and Android Pay. The essential two steps are writing to /system/build.prop in recovery and making the global setting change.

Do you not need to wipe dalvik/cache when dirty flashing nougat builds? OP says no wiping.

Use ext4 instead of f2fs, this is the most stable at the moment.
Installation
- Flash the latest TWRP version from the Recommendations section
- Wipe your device through TWRP
- Recommended Format /data as ext4 (f2fs is unstable at the moment)
- Flash the latest firmware from the Recommendations section
- Install the ROM from the download section below with TWRP
- Install GApps, (ARM64, 7.1, I recommend stock but a different package should also work) (http://opengapps.org/)
- Boot your device, done!
Updating
- Make a Nandroid backup
- Check whether there's a new TWRP release (from the Recommendations section) and in case flash it *before* going on
- Check whether there's a new firmware release (from the Recommendations section) and in case flash it *before* going on
- Flash the ROM with TWRP
- Wipe Dalvik / ART cache and Cache in TWRP

Not answering your question directly, but to make clear what this ROM is about. I added the following to the OP:
Goal of this ROM
LineageOS does not support the OnePlus 3T yet. The goal of this ROM is to provide a clean LineageOS experience for the OnePlus 3T (and also support the OnePlus 3). To support both devices, changes in 2 LineageOS repo's are needed. Comparison between my repo's and LineageOS:

- I can provide nightly builds instead of weekly builds
- I can add the commit from SultanXDA that bypasses the safetynet check
