Reinstalling youtube might help here, I don't have the issue myself, did you install youtube from the store or from a gapps package ?Is anyone else facing a reboot on using Youtube. Sometimes is gets stuck or forces a reboot. I have attached the logcat.
EDIT: It got stuck around line 1600 of the logcat.

I know that some Google sync issues were reported last week, might also have played a role.
Banks gapps don't include Google Now Launcher, that's why you don't have it, you need to modify the banks gapps to include it. Another way would be to use another gapps package, like Open Gapps.for some reason Google Now Launcher isn't listed at all in settings/home and I can't turn it on. Nova for example works normally.

The name of the apk would be velvet.apk in case you need it.

The bug you found is Gapps related, you might want to contact Banks team to see with them, officially, OmniROM doesn't offer support for Gapps, we only tend to recommend Banks because it has been proven that it's working well without being bloated. But then, it's nothing official on that matter. By the way, here's something interesting: in Ubermallow the Launcher3 icon is a green circle with a white house in it, and Google Now Launcher works fine there even with Banks gapps. In both Omni and OctOs the Launcher3 icon is white with a house with blue and red dots in it, and in both of those roms Google Now Launcher doesn't work with Banks gapps. Any correlation between the different Launcher3 icon and whether Google Now Launcher will work or not with Banks?
P.S. I think I found a bug - When I'm setting up the phone at the beginning and Google lists my devices it has backed up, when I choose the device and the apps that were on it that I want restored and click Next, it simply opens that page again and again. The only way to move forward is to click that this is a new device, and at that point the phone starts automatically downloading the apps I selected the last time before I chose that this was a new device.

Base camera is part of the ROM, but low light settings, Nexus 4 always was really horrible with it. But a black image with flash ? That one never occurred to me. I just did a test, in a dark room, and it works just fine, with both base camera and Google CameraI don't remember if the camera is part of the rom or gapp. I have problem with using flash in low light setting. Sometimes, It just gave a black image. Google camera from the play store has the same problem.
