I'm not on this rom...i just give u suggestion if it helps u..try it...goto setting then app section and find camera and enable all permission

Yeah, I have the same problem with every 8.1 rom... Now I found a way for a reproducable fix...
You have to restore a data partition. So you have to:
1. Install rom without gapps, boot up and do some settings, which you need.
2. go to twrp and safe data
3. wipe everything
4. Install rom and gapps together (you need to install both together, if not you will get a gapps fc)
5. wipe cache and dalvik and restore data!

Edit: It´s independent from sources: I tested Teammex, nichcream, TeamReloaded and TeamHyper´s sources... Everywhere the same!
