That's what I thought. The problems you describe are symptomatic of a dirty flash.

I'm having really strange issues with stability. For a while I was having really bad memory management where I couldn't use more than one app at a time (at the worst point even one app was crashing) even home would crash. Now I get GPU problems. The phone starts lagging really bad, especially during games, and locks up completely. Then says system UI crashed and goes back to normal. Sometimes I fix this by triggering a system UI restart, but sometimes even that doesn't work and instead makes my phone reboot.
I want to move to PD13 but the lack of customization (mainly status bar) is the reason I went back to PD12. I don't think PD13 is more stable anyway.

I know this may sound stupid, but...
Have you tried the factory image? If you're still having problems with that, perhaps your hardware is dying.
On the subject of the factory image, when you went back to 12, did you start with the factory image from Google?
If you have done those: Did you root the boot image? Install a custom kernel? Play too much with kernel settings? Install EdXposed? Install unstable Magisk modules?

PixelDust ROM is an Android 12 ROM inspired by the original Google Pixel firmware with full support for Material Design 3.
PixelDust CAF is based on the Code Aurora Sources, more commonly known as CAF. CAF ROMs are therefore generally considered to be more optimized for Qualcomm’s Snapdragon SoCs than AOSP ROMs (in performance, RAM management, battery, etc.).
