It's a zip file that has to be flashed by using TWRP after you flash Lineage OS 14.1. It modifies the device tree of the boot partition.

Stock N9002 ROMs and Oreo ROMs (so Lineage, RR, etc). Does not enable SIM2, but everything else should work.
