Icon pack is materialistic, kwgt widget for clock can't remember which as I have given the phone back. Its either ornate, twelve or odisea kwgt pack.
Here's mine using nova launcher and fluid navigation gesture so the animation isn't clunky!

I had the same problem, but i found out besides switching off the battery optimisation you also need to activate "home action compatibility" under "advanced" in the F.N. settings. Now it works perfectly.fluid navigation gesture stops sometimes and thats annoying
