Perhaps you have cm12 and installed an incompatible theme? I had this problem a few days ago, I installed that miui theme, every time I unlocked it it would restart

Rom / Kernel: Stock Modified?

Did you notice it after installing any specific app?

Factory Reset is typically your best bet.
