As mentioned, I also can recieve calls, but cannot make any. I'll either get a call not sent message or it will go to the call, but will just say "Call ended". The cell phone reception icon shows 1/3 of what it normally shows. Could the radio firmware be problematic?
Texting though, seems fine.

UPDATE:

Removing the back cover does not make a difference to the reception meter. Upon reboot, the reception meter shows empty, which is normal, then shows the "No SIM" icon, then goes 1/3 of the way.

Hi guys,

Its been long time I gave any update. I was really busy trying to get the Mobile network and calling working. I am still not there yet.
What i have found till now is that CM-12 Telephony code is highly dependent on ICCID ( aka Sim Serial Number ). According to my research this was not working for all kinds of SIMs or atleast a combination of SIM and our old libril_htc.
For now I have been able to make outgoing calls from my phone by hacking the code with some hardcoded iccid when code cannot get it from SIM.
To confirm my theory i need someone who is not able to make outgoing calls from CM12 to install CM11 and try the following apps and share screenshots.

Sooner someone can give me screenshot, the earlier i will be able to release something working.

Thanks guys, so your screenshots proves my theory. Now for now i should be able to fix Outgoing call by faking a SIM Number / ICCID.
Next thing to look is mobile network and proper fix for our iccid issue.
