I found the solution...
It was bricked, and I`ve managed a way to unbrick extRom (here was the problem).

The solution was realy easy...
using mtty you just have to "unflash" everything...
just folow the steps I got here from marleyb, but like this.
[1- press enter
2 - type "set 14 0"
3 - type "task 28 55aa"]
you shoul see now the program runing a lot of things that I don`t remember what...
then when it ends try the SD instal...
If It still doesn`t work then:
[1- press enter
2- "erase"
3- "checksum"
4- "checkimage"
5- "task 0"]
then reflash your radio & OS by SDCARD...
worked for me

hi everybody,
yesterday i was so silly *+# to try a custom rom flashing onto my treo 750v.
made HardSPL unlock - sim was already unlocked.
now i have the same symptoms described here - but no solution works.
treo hangs on boot with black screen and i am able to connect with mtty.
but both described commands (they work) do not help my treo!
is here any other solution possible? are there any docs for mtty commands?
i could shoot myself...

I`m sorry for my late reply, so is your treo working fine? but slow?
you can cook your own rom, or you can use the ones that are already here in the forum...
the ones here are fast and stable so you can use them!
but if you google a litle there are millions of things you will find to make your "original rom" to work a lot faster...
but I believe that if you learn how to cook a rom you will be able to choose what you realy want in your OS, so by removing everything that you don`t use and don`t need you will have a realy big change in the speed and of course you will have a lot free memory, and by cooking, you can put there just what you need...

I have the same problems as you. I am trying to install back the old ATT ROM that was working in the beginning. I am trying with the procedure above but those are the results:
It stucks in OS writing.
History:
What I previously done was:
1- I used Cheetah Unlocker and then I have been able to load up the WM6 Lite cooked Rom.
2- I have tried to load a more complete ROM and I did it wrong loading the HTC Vega 8 R0.
It was stuck in no Boot-Up and everytime I try to upload a new ROM it stops in OS part:

CHEEIMG.Nbh - FAIL
Splash1 - OK
Bootloader - OK
GSM - OK
MicroP - OK
OS- Fail
Upladte terminate.
Any help guys???

I am very very desperate. I have tried all the possible combinations to make it work and no success at all.
Starting from:
I used MMTY via USB and performed:
then I loaded from the SD card the CHEEIMG.nbh file recovered from the Italian official ROM: Treo759_2_30_ROM_IT_Desktopupdater.
result after around 5 mins of flashing from SD CARD:

My idea is that every each time it tried to write on the OS part it stops with the error 00018003
I have tried a cooked ROM with OS only no Radio or others and it stops at the beginning of the update!
What is strange is that it shows HTCSC after set 14 0 instuction. I loaded the HTC Vega 8 R0 image in the beginning that was dedicated for HTC rather for Treo! Can this influence the result?

Update...

Me too, I have all the problems that you described. And the greatest is when is use "task 28 55aa" command, the tero 750 goto down immediately.
Do you know Zhong Guanchun in Bejing of China? There are large numbers of technical personnels. I am back from there just now. I have asked too many people all reply me "cann't restore, except replace a chip."
I don't konw if our forum has a hero can deal with this problem use some software.

I am very fizz now. It is Beijing's time 4:25 AM. I will persevere to day break and go to Zhong Guancun take my treo 750 back lest they take apart my baby. haha~~~~~~~~~~
BTW, when I deliver 750 to fix I have buy a 8G iphone. After back to home I have upgraded it to 1.1.4 version, use Zihonewin 2.6 version unlock it, install Chinese font and input. All those works are finished in one houre. I feel the process of unlock iphone is very full-blown and professonal. It is very simple to people who have no knowlage about iphone just like me. You just read handbook, follow it, succeed at the end. Almost have no chance make it to brick. Iphone is wonderful, amused and beautifull. Good Apple.

The trick is in the Task 2A procedure.
The phone was updating until the correspondent percentage of the task 2a procedure.
I mean if I was updating until 100 Blocks with the task 2a the update was taken by the PDA until about 10%. If it was of 200 Blocks then until 20% and so on.
I am pretty sure about the above instance because i started and stopped the 2a procedure 2 times and the results along my tests were exactly like above!

Thanks for all the help... i had the same issue...... I tried to flash the original WM6 treo and i got OK on each step (Bootloader, Splash screen, GSM, Extrom, OS) and i still get black screen after i reset!!!!!!
But thanks to chaimd, rayjabs, superhiperclim and everyone.. the procedure (task 2a) is great followed with superhiperclim steps
