I went step to step.. I already have installed lineage 14.1 os so i just downloaded the modem and patch, went to recovery, installed the modem, then installed the patch and after wiping cache and dalvik cache i rebooted to system and nothing has changed...

3. Download the patch zip and flash it through twrp
4. Reboot and Enjoy ..
