This is mostly done by invisiblek. I am building and maintaining the post on XDA. I am not responsible if you brick your phone. CM12 is very early stages and YES it will have bugs!! We are working on getting these fixed. I cannot look in to issues unless you provide me with a log.

This is NOT a port! This is built directly from CM source.

Work Arounds:
No Data after initial flash - Go to Settings > More > Cellular Network > Access Point Names. Choose T-Mobile US LTE IPv6

Modem Crash - Toggle Airplane Mode or Reboot. If you experience this, please provide a logcat as soon as it happens.

Camcorder doesn't record - Update to 12/04 or newer. Go to settings and change the recording resolution to 1080 or 720.

Booting now. No issues with install.

[Edit] Booted and running great! Not a fan of the current DPI so I will be changing that but love it! No need to reflash for me until NFC works. All else works for me.

The icons are really small!! any idea how the dpi could be changed?

Edit the build.prop then reboot. I'm going with 640 dpi

[Edit] if you haven't already you'll need to flash SuperSU 2.23 beta. Them I downloaded Build Prop Editor in the Play Store and changed the LCD Density to 640.
