So I cannot get this too work. Do I need to write something to the NFC tag first? I have the Galaxy S4 and I had tried to do a NFC to unlock before and I couldn't get it to work.

Nexus 4
Doesn't work with the Nexus 4, i have both Broadcom Topaz 512 NFC's and an Ntag203.
When I scan the tags, nothing happens.
The Ntag can be registeret in the app, the Topaz can't.

Edit: Working with the Ntag203 now, weird. But still not working with the topaz..

I can read my Samsung Tectile, but when I get to the screen which I assume is the one where you enter a name for the tag, I cannot do anything. No keyboard comes up and when I hit back, there's not even an empty entry. Any idea?

Hmm, what happens if you hit Ok? It should be saved as an empty tag.

V1.9.1 fixes dialog issues on devices with a lower than 17 API level.

Your logcat doesn't help with the issue, mainly cause that's stuff from the system, not the mod.
Turn on debug mode and filter for Xposed.

Anyway, I think I figured out the issue. I'll come up with an APK tomorrow.
